Supplemental insurance is a collection of mini insurance plans that you can sprinkle on top of your regular health insurance. While not a substitute for Major Medical coverage-not (ACA) compliant, Premier Options is available for those who are unable to qualify for or afford Major Medical insurance. The Premier and Select packages are designed to include a variety of both inpatient and outpatient benefits, including a strong surgical schedule that can be accessed either in or out of the hospital an unlimited number of times! (Subject to a dollar limit, this varies by plan) Benefits can be paid to any hospital or doctor, but for those that wish to have additional savings; Premier Options also includes access to the MultiPlan PPO nationwide Network. If you use the providers in the PPO Network it offers additional cost savings. Enroll all year-round.

This is not an (ACA) exempt plan. Services: Provides a daily benefit option on confinement to hospital due to illness or injury. Subject to a 30-day waiting period for illness. Enroll all year-round.  Daily benefit per confinement: up to $1,000 *Only option available on Guarantee Issue.

Hospital Confinement Benefit 1-5 days: 100% of the daily benefit option-6-10 days: 50% of the daily benefit option-11-365 days: $100 per day. Intensive Care Confinement Benefit; (In lieu of Hospital Confinement Benefit) 1-2 days: 200% of the daily benefit option-3-10 days: 100% of the daily benefit option-11-30 days: 50% of the daily benefit option-31-365 days: $100 per day.

Waiver of premium benefit: After a period of hospital confinement for at least 30 consecutive days, this additional benefit waives the monthly premium, up to the maximum period payable, with no interruption in coverage. Lifetime Maximum of 365 Days.

DEDUCTIBLE PROTECTION- GUARANTEED ISSUE- FIXED BENEFIT PAYOUT #SM2

24-Hour, Accident and Sickness Hospital Indemnity Policy- Enroll all year-round. As major medical costs continue to rise, people find themselves taking on higher deductibles to help keep their costs down. However, this leaves you financially exposed if you are hospitalized or have outpatient services. Deductible Protection gives you the peace of mind that you need by providing you with a lump sum of cash if you are admitted to the hospital for 24 hours. Plus, there are additional outpatient riders that can be added to meet your specific needs. This is not an ACA exempt plan.

Guaranteed Issue for full-time employees ages 18-64-Benefits Paid in Cash Directly to You- Supplements High Deductible Health Plans-Build your own plan to meet your specific needs. Pays a Lump Sum Benefit up to $5,000 the FIRST time an insured is confined to a Hospital as a Resident bed patient during the Calendar Year due to a covered Injury or Sickness. Hospital Confinement is defined as being admitted to a hospital as a resident bed patient for 24 hours. Enroll all year-round.

Surgical- Surgical rider for a maximum of 3 days 50% if outpatient-Pays the selected benefit amount per day for each day of a period of continuous hospital confinement during which one or more surgical procedures take place not to exceed 3 days per period of confinement. For surgical services rendered in an Ambulatory Surgical Center, pays 50% of the selected benefit for the day the surgery is performed. Pays up to $3,000/day.  Emergency Accident- Pays the specified benefit for emergency care rendered within 72 hours of the injury by a physician in a hospital emergency room or physician’s office. Pays for up to four different covered Injuries (emergency treatments) in a calendar year per insured category (4 for employee, 4 for spouse and 4 for all children, not each child).

Outpatient Sickness- Pays the specified benefit for treatment in an out-of-hospital facility (including a physician’s office), due to a covered sickness. Pays one and one-half (1.5) times the benefit selected per sickness for treatment in a hospital emergency room. Pays for up to four different covered sicknesses (one treatment per illness) in a calendar year per insured category (4 for employee, 4 for spouse, and 4 for all children not each child). Intensive Care Unit- Pays the daily benefit amount selected for each day of confinement for which there is a room and board charge by the hospital in a Hospital’s Intensive Care Unit for a covered injury or sickness. Pays up to $1,000/day. This benefit is payable for up to 31 days for any period of confinement.- Diagnostic- will pay the benefit amount indicated on the Policy Schedule when any covered person incurs charges for and has one of the following diagnostic procedures while this policy is in force. This benefit is payable for one procedure per calendar year per covered person. If you have one of the covered Diagnostic Procedures which would be payable under the Outpatient Surgical Procedure Benefit, we will only pay the Diagnostic Procedure Benefit. Pays up to $1,000. The procedure must be required due to a covered accident or covered sickness.

OUT OF POCKET PROTECTION PLAN- FIXED BENEFIT PAYOUT #SM3

Helps pay deductibles and copayments. Your choice of benefits and premiums. Pays benefits directly to you unless assigned to help with hospital bills and out-of-pocket costs. Pays in addition to all other insurance and workers’ compensation. Benefits are paid in a lump sum directly to you. This is not an ACA exempt plan. Daily Inpatient Hospital Confinement Benefit with Doctor Office visit: (per hospital admission) you may choose a daily inpatient benefit among the options if you are confined in a hospital as a resident inpatient. Plan pays the daily inpatient benefit you select up to 10 days per hospital confinement. First Hospital Admissions: (1 per year) you may choose an admission benefit of up to $6,350

Outpatient Surgery Benefit (2 per year): you may choose a daily benefit of up to $3,000- For surgical services rendered in an Ambulatory Surgical Center or Outpatient Hospital Facility, plan pays the amount shown on the Schedule of Benefits for the Outpatient surgery performed- Emergency Accident Benefit (4 per year): $250 if you sustain an injury which requires emergency care by a physician, We pay the actual expenses incurred up to the amount per treatment you receive in a hospital outpatient emergency room or urgent care facility. The treatment must be received within 72 hours of the injury. Enroll all year-round.

FIXED INDEMNITY DIRECT UP TO AGE 83- FIXED BENEFIT PAYOUT #SS2

Benefits include: hospitalization up to 365 days per incident paying up to $3,500/day, surgery, ICU paying up to $6,000/day up to 30 days, outpatient surgery, ER, outpatient services, ambulance and doctors visits. The money can be used to pay unexpected medical costs or everyday living expenses. Benefits are paid directly to you - not your doctor or hospital. METAL GAP - INDEMNITY PLAN- FIXED BENEFIT PAYOUT #SI1
